View your health records online

If you’re a Montage Health patient, you can access your electronic medical records using an online tool called a patient portal.

MyChart is the newest electronic health record available to patients of Montage Health and affiliated providers. Use a free MyChart account to access your personal information and take care of other common tasks, such as bill pay and communicating with your Montage Medical Group provider.

Central Coast Health Connect account

Central Coast Health Connect (CCHC) is an established community electronic health information exchange between Montage Health, Natividad Medical Center, and Salinas Valley Health. If you have a community Health Information Exchange (HIE) account through CCHC, you can view test results and other parts of your electronic health record.

Permission to release your records

You'll have to sign an authorization (permission) form before we can:

  • Give you copies of your medical records
  • Send your medical records to another provider or facility not affiliated with Montage Health

How to release and request medical records

Send your information to a provider or request copies of your medical records from Montage Health through the following process:

  1. Download our medical records request form
  2. Print the form
    • If you don’t have a printer, you can get a printed form at the front desk of the Health Information Management office. Their office is located at Carmel Hills Professional Center. The physical address is 23845 Holman Highway, Suite 307, in Monterey.

  3. Complete the form
  4. Send the form to Montage Health by:

When will I get my medical records?

After you submit your request form, expect a Community Hospital staff member to contact you to let you know when your records will be ready.

Amending your medical record

Make a change to your Montage Health medical record using the Request to Amend Protected Health Information Form [PDF].

Complete the form and then mail it to:

You may also deliver the form in person to the Health Information Management office office at Carmel Hills Professional Center.

Health Information Exchange

Health Information Exchange (HIE) allows doctors, nurses, pharmacists, other health care providers and patients to appropriately access and securely share a patient’s vital medical information electronically—improving the speed, quality, safety and cost of patient care.

Opting out prevents your personal health information from being shared, with two exceptions:*

1. The opt-out does not apply when the disclosure is made to public health authorities and the purpose of the disclosure is permitted by either HIPAA or applicable California law.
2. Your information may be disclosed to facilitate emergency medical treatment.
